Release of Records TO Center for Maternal-Fetal Care This form is for you to complete if you want records sent from another provider's office to Center for Maternal Fetal Care. Please be sure to send the form to the provider you're requesting records from once you have completed and signed it. Release of Records FROM Center for Maternal-Fetal Care This form is to request that records from Center for Maternal Fetal Care be sent to another provider. Blood Sugar Log Please complete this blood sugar log and save it to your computer. You will then want to go into "Ask Our Staff" through the website portal and attach your saved form. Please make your subject line "diabetes" and attach your completed form. The nurse will review your log and respond to your message.
